Services We Provide
Nursing Services
We provide Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to look after your loved ones’ medical needs. They will coordinate with our interdisciplinary hospice team to ensure their care plan is strictly implemented.
Hospice Aides Services
Our hospice aides will assist your loved ones with their daily living activities. We will make sure that they are safe and comfortable without taking away their sense of dignity. The services provided by our health aides include but are not limited to:
- Personal hygiene assistance
- Grooming assistance
- Meal preparation
- Light housekeeping
- Toileting assistance
Spiritual Counseling
We will assist your loved ones and your family in coping with the challenges brought about by a life-limiting illness through spiritual counseling. Our chaplains and spiritual counselors will help ease your spiritual distress and frustrations. We will make sure to consider your family’s beliefs and act as a liaison to your religious community when necessary.
Social Services
Our hospice social workers help families cope with the stress brought about by a life-limiting illness. They can provide social, psychological, and emotional support when needed. Our social workers can help families with care coordination, community support, and resources. They can also help in handling the necessary paperwork associated with hospice care.
Dietary Counseling
Proper nutrition is necessary to ensure the quality of life of patients with life-limiting illnesses. Our registered dietitians will coordinate with your loved ones’ attending physician and nurse and come up with a personalized nutritional plan.
Physical, Speech, and Occupational Therapy
As much as possible, we want our patients to maintain their strength and functionality. If their health conditions allow it, our therapists will coordinate with your loved ones’ attending physician and create a therapy schedule to improve their vitality.
Pain Control and Symptom Management
We know that pain relief is what matters most for patients with life-limiting illnesses. To ensure that your loved ones spend their remaining days in comfort, we provide pain management services. These include the administration of pain relievers and the implementation of pain relief techniques allowed by their health conditions.
Hospice Medications
Taking maintenance medicine is essential for a good quality of life for patients with life-limiting illnesses. Our caregivers will make sure that your loved ones are taking the right medication at the right dosage and on schedule.
Home Medical Equipment and Supplies
Merit Hospice will cover all the medical equipment and supplies needed to ensure that your loved ones will be comfortable at all times. You no longer have to worry about purchasing them because they are already included in our hospice care services.
Inpatient Care
When pain and symptoms can no longer be managed at home, patients can be transferred to an inpatient facility. Inpatient facilities include hospitals, assisted living facilities, and nursing homes. Patients who are taken to these facilities are still eligible for our hospice care services. We will coordinate with the inpatient medical staff to ensure the quality of life of our patients.
Continuous Care
When patients experience severe symptoms brought about by their terminal illness and choose to stay in their homes, Merit Hospice can provide them with continuous care. With a physician’s assistance, a nurse or hospice aide will supervise them 24/7 until their health condition becomes stable again.
Respite Care for Caregivers
Caring for a terminally ill loved one is tough for a family caregiver. That’s why we offer respite care services to give family caregivers some time off without compromising the welfare of their loved ones. We offer short- and long-term respite care depending on your and your loved ones’ needs.
Bereavement Support
Losing a loved one is one of the most painful events in life. When the pain and grief become too overwhelming, you can count on our bereavement coordinators and counselors to help you get through this difficult time. Our services include individual and family counseling to help you healthily cope with grief.
Volunteer Assistance
